I was having the same troubles, this is what I did to get it to work:
host: omnidc08.corp.omniamerican.org
Base DN: "DC=corp,DC=omniamerican,DC=org"
Administrator DN: "CN=Chalkley\, Jeff,OU=IT Operations,OU=Information Technology,OU=Back Office,DC=corp,DC=omniamerican,DC=org"
